AUC24 vancomycin Bayesian-based Dosing: Increasing Therapeutic Target Attainment with Decreased TDM cost

We compared therapeutic range attainment across five hospitals using vancomycin trough-based dosing versus five hospitals using Bayesian-supported AUC24-based dosing. DoseMeRx enabled significantly higher target attainment with fewer drug concentrations and lower mean doses, suggesting reduced AKI risk and avoiding added lab cost.
